Collecting patients from their homes and transporting them to and from the Hospice. 1 to 3 hours per week (Tues, Weds and or Thurs)
Main tasks:
· Collecting patients from their homes on a prearranged day and time and bringing patients to the Hospice
· Handing patient onto Day Hospice staff
· Returning patient back home at the end of their Day Hospice session
· Ensuring patient gains entry to their home
· Referring all medical, social and personal problems expressed by patients to Day Hospice staff
Experience and / or skills:
· Have your own transport
· Have a clean driving license
· Have careful and considerate driving abilities
· Be flexible
· Be reliable
· Have good time keeping
· Have good communication skills
· Be attentive
· Be a good listener
· Show empathy
